[SOLVED] Is my SSD dying at 70% health?

I've had my SSD (Samsung SSD 960 EVO 500GB 500.1 GB) for about 2 years now, recently my pc has been crashing and my disks have been spiking to 100% while playing games. I'm not much of a tech-head and I don't know the ins and outs, but I've done multiple stress tests, defrags and searches and nothing seems wrong.
Can anyone look at this CrystalDisk screenshot and see what's up because I don't know how to read any of this, thanks!

High number of error information log entries but everything else looks good. 28 unsafe shutdowns however. There are tools to check the logs although who knows how informative that will be. Health seems good.
