Question Is my video card dying on me?

Motherboard: MSI B450 Gaming Plus Max
Ram: 16GB
SSD/HDD: 1TB HDD
GPU: 1050 GTXTI
OS: Windows 10
Monitor: LG TV 32LH510A-TA



I used my desktop PC about 2 weeks ago and had no problems. Its been running a bit slow so have been wanting to do a clean format for a while. This morning, it was slow so i removed all unnessesary software to see if that would make a difference also disabled some startup programs. After rebooting, i started seeing graphical glitches, like pixelations. I figured something i installed messed something up and since i wanted to do it anyway, I did a reformat and reinstall of Windows.

Everything seemed fine, i updated nvidia drivers and the board msi drivers. Then noticed the problems again. pixelation and just crashes/freezes. I did another clean format/install and this time just gave it time without installing any drivers but happened again even without nvidia drivers.

The monitor i hooked up to is fine, and i also changed hdmi cables. The issue happens randomly, not while using anything specific. When i had the nvidia drivers i used a free benchmark software and didnt have any issues. Dxdiag also didnt return any problems. The installation worked just fine without freezing and I also spent a chunk of time in safe mode with no issues. I've attached screenshot of what the pixelation i am talking about looks like. Could my card be dying on me?

When posting a thread of troubleshooting nature, it's customary to include your full system's specs. Please list the specs to your build like so:

CPU cooler:
Motherboard:
Ram:
SSD/HDD:
GPU:
PSU:
Chassis:
OS:
Monitor:
include the age of the PSU apart from it's make and model. BIOS version for your motherboard?

Go into Device Manager and see if your GPU is flagged with an error + Use GPU-Z and pass on a screenshot of what you see.
