ISOBAR surge protector FAULT and LINE OK led's on simultaneously

I just got a Tripplite ISOBAR Ultra surge protector. This one

And just as I thought there is something wrong. The wiring in my house probably has been messed up all my life though never had any issues with my electronics (so far).

The FAULT led and the LINE OK led are on at the same time. PROTECTION PRESENT led seems to be dimmed or off.

I thought FAULT and LINE OK were opposites (one indicates the outlet is fine, the other indicates the outlet is not fine) how come they both are on at the same time.
Same results no matter what outlet I plug this thing into.

What are your thoughts? Did I get a faulty piece or is my house wiring bad?

To rule out your component as faulty you're going to have to do some leg work and take it to your friends place or at least a neighbors house where you know they don't have an electrical problem. There you can test out your surge protector and see if the same goes for their house. The next step would be call over a certified electrician and check out the wiring and power in your house if the surge protector doesn't exhibit any issues at the test house. You've already established that it's doing the same thing on every wall outlet in your house.

The last thing to do if the electrician says your in the clear, then the surge protector is the culprit. RMA /refund/exchange it.
