Issue with graphics?

CPU Build:
Processor: I5-8600
Graphics card: ZOTAC GTX 960
PSU: Seasonic Focus Plus 550w GOLD
RAM: Corsair Vengeance LPX 2666Mhz (2x4GB)
MOBO: MSI H370M Bazooka

The ZOTAC GTX960 is a refurbished one i claimed from ZOTAC under warranty!

However, after playing CSGO with low settings for about 4 hours, this showed up.

Should i be certain its a problem with the graphics card? When the screen becomes like that, the only way to handle it was to power off my pc. Thereafter, i start CSGO again and moments later this happens again.

Kindly appreciate any help as I'm not too familiar with these.

Edit: I've already updated the relevant drivers thru GeForce Experience
Edit2: I tried running heaven benchmark on it and it stopped at 72 degrees and this happened

Looks like a defective GPU, overly sensitive to heat, candidate for an RMA.
Before RMA GPU, I would be reseating GPU in it's slot, with no major sagging or wobbly PCIe slot, checking PSU power cables attached to your card, and monitoring it thoroughly using GPUz while idle and under load, also you could try adding extra fan close to GPU to help cooling it and see if any improvement is noticeable temperatures wise.

Solution
CSGO at low settings on that system? The framerate must have been very high. That could cause the card to heat up.

If possible I'd test it in another computer. If it does the same then you know it's the videocard. Alternatively, what if you ran a game with the framerate capped at 60fps? Or at 30fps? The idea is to see if high framerate/high temps are what's triggering this. It might need new thermal paste.
