Issue with my new GPU???

Hey guys! I'm at wit's end here! I got a new GPU for my PC, it's the XFX GTR Radeon RX480 8GB Black Edition, and it's a great card! But when I play an intensive game for about 10 to 20 minutes, my screen goes completely black and the GPU just out right fails! Forcing me to shutdown my PC and restart it. I have no idea what could be causing this, I installed a 750W PSU before hand so I don't think it's that, some have suggested it could be the driver, but I made sure to install the newest AMD driver for it! Ideas? I'll give more info of course if needed!

Mobo: Gigabyte GA-78LMT-USB3 Rev. 6
RAM: 1x 8GB DDR3
CPU: AMD FX-6300 6 core

It was The Forest, and Rise of the Tomb Raider. Only games I've tested on it so far, both have crashed the GPU.

PSU is an EVGA 750GQ Gold

What other info do you need?

Ran both games at ultra settings, but this GPU passes recommended for both games.

Stock cooler on the CPU and GPU, the GPU stays at about 80C but piqued at like 95C (it's pretty high) and the CPU is at about 55C. Think it could possibly be a heat issue?

if the gpu peaked at 95C that is very high Kinda something to be a bit concerned about. the CPU temp is very nice. But yes it looks like the problem is your GPU, something with the cooler must not be working right. Could be dusty could be something hindering the fan could even be the thermal paste on the GPU you should really look into it a bit and see what it might be exactly.

I have a EVGA 1070 and it never gets above 65c at full load so something seems a bit off on your GPU, I'm not sure how hot the 480's are suppose to get but 95 shouldn't be acceptable. 80c is fine but if it sits at 95 for a bit that's concerning.

Do you know of a good software to show temps while in game so I don't have to keep ALT+TAB? I'm really unsure of what it could be, but heat is a possibility. Thanks for responding!

UPDATE: I ran some games with my case laying flat and open, and it shows something like, 72C most of the time, significantly cooler! As you can see in this pic, I have 2 fan slots on top, would filling those with some good fans possibly solve my issue?
