LoL Error 4101 every game.

I am experiencing a very weird error that I keep getting over and over, however, it open happens when I am IN GAME on league of legends. The screen will randomly go completely black for anywhere from 5-15 seconds and then come back. These crashes can happen anywhere from 1 or 2 times a game upwards to 10 or 11.

What I have done so far to fix it:
Reinstalled windows 3 times.
Reinstalled league of legends 5+ times.
Full repaired league's files from the client.
Tested my RAM and graphics cards and there don't seem to be any errors.
Tried TDRDELAY multiple times with no luck on it fixing the problem.
I have tried overclocking my GPU in hopes that it would push past the problem but no such luck.
I have uninstalled my graphics drivers 5+ times and reinstalled them with no such luck, I also used DDU to make sure that they were completely off my computer.
I have lowered the graphic settings in League itself and it made no effect on it.

I submitted a ticket to RIOT GAMES and talked to their support team over the course of the week, after completing all the tests and giving them all the logs they needed they said that it was related to my internet and I was losing packets. I am not sure how that would make my graphics card crash and recover because the two are unrelated. Not only that but I have a consistent ping and get no lag or lose connection at all.

AMD FX 8350 Black Box Edition 4.0ghz
Radeon R9 FURY 4GB
8GB Crucial DDR3 RAM x1
MSI 970 Gaming Motherboard.
Thermaltake Smart Pro 750w PSU

Any help would be much appreciated, I have been ripping my hair out trying to fix this over the last few months. Thank you and let me know if you need any other information.
 
Solution
Could be your game is locking up due to lose of contact with the game server. Check your ethernet cable or wifi for packet lose by pinging your router. Make sure your router has the games ports opened and forwarding. Check your firewall for issues blocking the game.

Problems of never ending Matchmaking Queue or Entering Champion Select

Forwarding Ports:

Each router is differant so you may need to check your router manual.
Obtain your Default Gateway and Internal IP Address:


Press the [Windows Key] + R
Type cmd and hit Ok
You should now see a black window labelled Command Prompt that is black with white lettering.
Type ipconfig
Hit Enter
You should now see a list of information in front of you. Write down your Default Gateway and IPv4 Address we will need these in a later step.

Access your router's configuration page:

Open your web browser.
Place your Default Gateway into the URL Address bar. (example 192.168.0.1 or 192.168.1.1)
It may ask you for a password and username if you set one.
Locate a page with one of these: Port Forwarding, Virtual Servers, NAT or Applications
Port Triggering is NOT Port Forwarding. It is different.
If you have reached the proper screen you will notice that you can input several pieces of information.
The correct screen will have these: A SINGLE Port Range, Protocol and Local IP/IP Address/Computer.
Now you must create an entry for each of the port ranges. It is very important to input the correct Port Range, Protocol(UDP or TCP), and Local IP.

Confirm your ports are forwarded:

You can confirm your ports forwarded corretly using this third party website: http://www.firebind.com/
Simply check the ranges above are currently open. If they thent double check you included is correct ports.
