Hey guys i was looking to buy a new graphic card for my PC. This PC is about 6 years old and the graphic card is dying! I tried cleaning, replacing the thermal paste etc., But It's unfixable! Here are my specs:

CPU: Intel Core 2Duo E7400 2x 2.80GHz 3MB cahce
GPU: nVidia GeForce 9500 GT 512mb DDR3
PSU: 500W
RAM: Installed 4 GB ( 3 GB usable )
Storage Drive: HDD 160 GB SATA III
Motherboard: ASUS P5KPL/1600
OS: Windows 10 Pro N x64 bit ( Activated )

If you can tell me a good graphic card that will go into my rig and also won't cause bottlenecks in games. I am always active and don't matter the cost of the GPU or anything just give me the name.

Older H110 motherboards can have compatibility issues with these newer pentiums. B250 or H270 would be the optimal, newer, option. If you can find one of these for the same price, go with it. Otherwise, try to make sure your H110 selection is updated for the g4560 pentium.

If cost isn't a determining factor, would you also like mobo/cpu upgrade recommendations?
Is your case a normal sized case or a prebuilt small form factor?
My first go-to ideas are the GTX 750 Ti, or the GTX 1050 mini/OC. There's some other Radeon cards too. I think a card around $100-130 would be the max before that CPU would just be bottlenecking it too terribly.

Lastly, could you give us the model of your PSU? The amperage is important to choosing a suitable card.

yo i wouldn't go for that high spec GPUs like the 1050, but i'll se if 750 is good enough, also my on my PSU it says AC Input : 230V 50HZ 3.5A
My PC case is MSI SIRIUS II MIDI TOWER, and also can you give me a CPU+GPU upgrade for my pc, that would be nice :D

We would need to know what it says for the 12v rails.

Like on this example, where it says +12V1 18A (this has 4 12V rails in the picture). We need to know how many rails there are and the amperage on them all.

And okay. The 750 Tis are about the same price as some 1050s now, and aren't as powerful. If they're the same cost, I wouldn't rule out 1050s. Here is what I used to upgrade a Dell with an i3 I had.

R7 250 is a cheap and decent card for old dual cores.

I would go for a 750 Ti or a 950. There is other Radeon cards out there that are suitable but I've just always preferred Nvidia. 750 Ti's aren't expensive at all so it shouldn't be a struggle. Are you happy with the performance of your current PC? We're getting around the part where you may need to upgrade other parts in your PC to be suitable or if your old ones start to break. I can give suggestions on what else you may want to upgrade in the future?

If you were to upgrade your CPU, you would probably need to upgrade your MOBO as well. If you were to do that you would probably need to change your Ram too because modern MOBOs are not DDR2 to that would have to change as well. You might want to upgrade your PSU too because since it is 6 years old, there is a chance of it giving way when you out in new parts so here is this list.

CPU - i3-6100 or Pentium G4560
MOBO - H110 - There is a lot of different makes of this board, they all come with their own perks. Choose one
RAM - 2x4gb Kingston HyperX
GPU - 1050 Ti
PSU - EVGA 600B - Its only Bronze Certified and isn't fully modular but it's not going to break the bank.

If you want to keep that motherboard then consider upgrading the CPU to a Core2 Quad Q9650 @ 3.0GHz. This would offer better compatibility with modern games that need more than 2 cores/threads to run at all.

Even without overclocking, the CPU performance will be more than double compared to the Core 2 Duo E7400.

You will need to replace the stock cooler if you do this. For moderate overclocking (say under 4GHz) this will do the job http://eu.coolermaster.com/uk/cooling/viewallthermal/hyper-tx3i/
