Low budget upgrade for ATI Radeon HD 4350

I'm looking to upgrade the graphic card on my PC. I do some amateur 3D modelling and I play League of Legends a lot, but I keep getting some slight lag on the game even at lowest quality. I do not know anything about all of this so I will just copy here the info that I find. Note: the computer is one of those slim office style computer.

Gaming depends mostly on the GPU and the CPU. You are hurting on both counts. And having a slim line case will limit your choices of upgrading the gfx card to a low profile design. If possible at all. The tiny PSU is another limitation. The items I highlighted above pertaining to the PSU (power supply) are its weak spots for a better gfx card. What is the exact make/model of the PC you are using? Apparently it is a Dell...

ur cpu and gpu arent very up to date for alot of games, even league of legends. depending on what cpu socket ur pc uses i would upgrade ur cpu and mobo first.

With a tiny +12V rail rated for only 15A, I wouldn't go much more than the GT 730 DDR5 64 Bit.
If this is what the insides of your Optiplex 745 SFF looks like, you have another issue. The gfx card must be single slot and very narrow. And obviously, low-profile.
