I'm using an Asus 144hz ASUS VG248HE, with a Titan card and i7-3770k.
At 144 hz I only get 25fps in far cry 4.
Is this normal?

with a titan you should be getting way more than that. i know nvidia have been having issues with there drivers and 144hz displays.
if you can try downlocking the monitor to 120hz (windows display control) and see if you get an fps bump.
also reset your nvidia control panel to default and if needs be remove farcry 4's nvidia profile from the supported games list.

No, triple buffering, v sync, and anti aliasing are turned off.

If i turn anti aliasing on to any of the provided options, my image is blurry. Nvidia suggests HBAO+ or something...
image is blurry.

Well that is strange the titan is more than enough to pretty much max far cry 4 out at over 60fps 1080p. Are you sure you have the latest graphics drivers installed?

Now I'm getting 60 fps. Let me try at 144hz ..
I can also try with dual titans

At 144 hz now I'm getting about 30 fps.(with one titan)
Anti aliasing must be turned off, otherwise the image is blurry.
Ambient occlusion and triple buffering do not make any difference to the image.

yeah it sounds like you have the nvidia driver issue. it will likely be worse if you try 2 titans at 144hz.
if its safe to run the monitor at 120 then thats what you will need to do. to find out if its safe to constantly run the screen at 120 you may need to contact asus.
i have seen a few posts of the same issue with guys that have 144hz displays ranging from low fps, to screen colour distortions and flickering.
