Mail in Rebates?

I am trying to get a $30 rebate on an ASrock product (keep in mind i have not yet purchased this item, just making sure i know what to do before i buy it) but the instructions are unclear to me. It says to "cut out the original model name" and serial number. Can someone help me out?

There's a label on the box that you cut out and send in with the rebate.

You cut that off the actual ASRock box the thing comes in.

Its called a UPC code. Bar code that you cut out and send in.
