Memory card not deleting anything nor pasting anything.

i have a samsung sd card of 16 gb. i have been formating it through command prompt as well as other ways. but after deleting all files and removing it from my pc all deleted files are again showing on phone and Pc. nothing is delete from the card. another thing is that after pasting files it is showing on Pc or phone but passing a few seconds all pasted files are varnished. please suggest answer.
