memory card strange problem

memory card reader can read the card once but after inserting in mobile asking to format... after that the card is unreadable.
i changed the card reader, many cards, and even mobiles.... still the same and also formatted the pc

check to see if theres a firmware update for it. if its from a dell check dells site first as sometimes they use their own modified firmware. If the existing hdd firmware starts with a D then its a dell, seagate usually starts with a j.
