Monitor has 1 HDMI and 1 D-Sub. Need help with computer and PS4 set up please

As stated, I have a monitor that has one HDMI port and one D-Sub port in the back. It's a 60Hz 1080p monitor. I'm looking for a way to hook up my PC as well as my PS4 to the same monitor without really sacrificing much since I use both for gaming.

My GPU has two DL-DVI-D, one HDMI, and one DisplayPort for hookups. Any help on this would be much appreciated! If more info is needed I'll gladly provide it. Not really sure what all is needed to know :/
