Question Monitor Inactive No Signal

I have a HP 25es. It's always worked perfectly fine, but rarely the display will go black and say "No Input Signal Found" and will say my HDMI is inactive. The only solution I've found is a hard restart.
Like I said, this has always happened rarely until i started playing the New World Closed Beta. It happens after just a few minutes of play. Every other game and program is fine. Only New World will cause this issue.
I'm not at all tech savvy, so I've no idea where to even start. I've changed HDMI cables thinking that may have been an issue, but it didn't help.
I lowered the game resolution and graphics to low and still does it.

GPU is a Radeon Rx 480.

Copy/Paste from the Amazon page. I've owned for about 3 years. Only change from the list below is I replaced the RAM with two 8gb G.SKILL RIPJAW.
Also, I'll add to my original post that when this issue happens I can still hear all sound from games or programs that are running.
All I can find on my PSU is LITEON 100-127v 10av50-60hz
The rest is all in chinese/Japanese.
