"Monitor out of range" issue popping up repeatedly

So, yeah, this issue keeps happening. It was happening about every 10-15 minutes, but it only did it once since then, so I dunno what to do about it. Running AdwCleaner seemed to fix things, but I guess it didn't.

It started happening after I installed Battlefield 4 on Origin. I didn't change the settings of my GPU or change the refresh rate or anything, I'm just using the computer normally, and all of a sudden, bam, "Monitor out of range".

Is there some sort of solution I could get? This kind of prevents me from playing games, as restarting seems to be the only way to get out of it.

Hi Zen,

It would help if you listed what graphics card you are using, its brand name, and the model number.
Plus the GPU chip it uses Nvidia or ATI.

Also the screen size of your monitor, what brand name it is and what the maximum resolution it can display at.
And the maximum Hz range 60Hz 120hz or 144Hz.

Also what version number of the video driver you are using.

Driver version: 365.19

Monitor: Asus VH238H Black 23" Full HD HDMI LED Backlight LCD Monitor w/Speakers (I think it only goes up to 1920x1080, and it can't go higher than 60 Hz)
