Monitor randomly crashing to solid color

I've been having a problem when playing League of Legends; between 5-30 minutes in, one of my monitors (even the one not displaying the game) will crash to a solid color. I haven't had this happen while playing any other, more graphically intensive games so I'm super confused as to what this is.

I've had this issue even after a PSU change, and the issue persists after resetting the monitor and plugging it back in, etc. so I'm assuming it's a GPU problem?

I can't find any errors in Event Viewer or any crash logs in WhoCrashed so.. if I could get any advice here that'd be much appreciated

Try use that one (the bad ?) for the main display w/o other monitors, and play the game again. If the monitor/pc will crash and you maybe have the error message in the event log, and from there you may find out what is that.
