Question Monitor randomly loses video signall, PC will still run

Sometimes, my monitor will lose video and my rgb keyboard won’t light up anymore.
Literally the only way to fix this is to hard reset the computer. Even when i do that, the problem will still occur.
The computer will still be on too.
I have already cleaned the RAM sticks and used 2 different HDMI cables.
The HDD LED light also turns off as well.

Gigabyte A320m-S2H
Ryzen 1200 AF
ThermalTake Smart 500W 80+ White
Patriot Signature DDR4 8GB (4X2) 2666 MHZ
Seagate 500GB 7200RPM HDD
Kingston A400 120GB M.2 SSD
XFX RX 560 4GB

The monitor is a Toshiba Model 23L1350U

Have you tried reinstalling your GPU Drivers?

Does it do this with other monitors?

i dont have another monitor and yes i have already reinstalled the drivers for it.
also, before it loses video, it will show some green rectangles for a few seconds and will play some weird audio. When it loses video, i can not reboot the pc so ill have to hard reset it
