Monitor Screen Goes Black, Sound Continues Playing for a Few Seconds then Loops Indefnitely

Hi there.
I recently built a brand new pc and have been encountering this issue since day 1.
As the titled states, my monitors screen goes black and any sound that was playing continuously plays for a few seconds before it goes into a glitchy loop indefinitely. Only way out of it is to turn it off at the case.
I havent received any form of error messages from it unfortunately, although i once heard Windows pop-up noise when it black-screened.
The crash itself has happened when gaming (playing Papers, Please), watching videos on YouTube and once or twice when idling.
Have attempted a number of different troubleshoots so far:
- Reinstalled Windows (according to Microsoft, it was a compatibility issue with PSU (???))
- Checked power cables to all components several times, including making sure they are all plugged in.
- Stress tested GPU and CPU and it never crashed - Recently installed a new fan to improve airflow although it made no difference.
- Monitored temps of CPU and GPU and they never went that hot at any point
- Also underclocked GPU as someone suggested it, made no difference
The only form of error message i have received is from me shutting the computer down at the case, message is along the lines of:

Are you sure your BIOS and remaining device drivers are up to date? Try uninstalling your GPU drivers using display driver uninstaller and then reinstall using the latest drivers from AMD. Make sure your ram is running at 1600MHz within your BIOS.

Are you connected to the internet with both the LAN port and the wireless card?

Have you ruled out your installer being corrupted by recreating your bootable USB installer? Is your option for automatic device driver updates disabled? For added measure did you follow up with this guide while building your PC?
