Question Monitor turns off, GPU Fans turn off. GPU won't work until replugging cables.

Hello, I'm having an odd issue with my GPU at the moment.

My issue occurs while playing graphics intensive games. After around 10-30 minutes of gameplay, my monitor turns off. I'm still able to hear a video or audio for around 45 seconds until it cuts off as if I've blue screened. When I look at my PC, everything is normal, all RGB, fans, etc... works. However, my GPU fans are not spinning. Even if I restart my PC, they do not start up. The only fix I've found is to replug my PCI-E power cables.

Could this potentially be a PSU issue?

I've also looked at my windows crash log file, which shows (on WhoCrashed)

I've tried doing a complete, clean reinstall of my AMD drivers.
I've had my GPU for about 3 years, and my PSU for even longer (probably about 6-7 years)

GPU: Sapphire Nitro Radeon R9 Fury
PSU: Corsair TX 850
CPU: AMD Ryzen 1600
MOBO: MSI X370 GAMING PLUS
OS: Windows 10
