monitor won't light up - need help *updated

A week or so ago, my computer began to have some video card issue. The monitor would not light up even when the system normally booted. Recently, the situation became worse. It won't boot no matter how many times I pressed the button. If I first pressed the power button, it stuck on 92 code (PCI initialization). I pressed again to boot, and it stuck on D6 error -No Console Output Devices are found. Sometimes, it can fully booted, and then it works fine.

I reset the CMOS, but not working.

Moved card to another PCI slot. Similar issue. The system was up. I heard the windows loading music and even my Skype login sound, but the monitor showed no signal.

System works fine when I unplug the video card, and use the integrated video card from CPU.

Here is my configurations. Recently I upgraded the mb and cpu. It worked smoothly for a month. I really have no idea what triggered this.

I did some research. It means the system cannot detect my card. But I don't know which component has issue, video card, motherboard or something else?

Video card RMA back. They did not find any issue with the card. They did 48h stress testing.
I installed the card back and have the same issue as before. So frustrating.
Maybe there is some compatibility issues with motherboard/vga card/monitor?
