Monitors going black when playing games in full screen.

Hi, I am currently having an issue with playing games in full screen on my computer. Sometimes (about 5% of the time) when a launch a game that is full screen it will cause all three of my monitors to go black and not function. This same issue also occurs when I alt-tab out of a full screen game (still about 5% of the time). My computer still is working properly because I can still hear game sound and talk to my friends on TeamSpeak or Skype just fine. I am running a 144hz monitor as my main monitor and the other two are both 60hz. I should also note that this issue does not happen when I play games in windowed mode or borderless windowed mode. Any and all help would be appreciated.

I imagine it goes black if the resolution of the game is bigger or smaller than what your monitor can display, as it goes "out of range" of what your monitor can actually process, that's why it doesn't happen in windowed/borderless windowed mode.

Does like, powering the monitor on/off or unplugging it and plugging it back in fix the issue?
And are you using eyefinity/nvidia surround? if they're trying to run at incorrect refresh rates that might cause it.
System specs?
