Question Motherboard Voltage Check

Been having an issue with my PC, mostly when launching games. Both monitors go black, and the speakers emit a steady tone. It isnt a full crash, if youtube or music is playing, it will continue to play despite the black screens and tone. Originally thought it was unique to launching Diablo 3, and began my troubleshooting on the Bnet forum, but have since observed it on other games/activities. Was suggested to be that it may be a voltage issues, but I'm not really sure what I should be looking for. Here are my current voltages with nothing really going on in the background.

Gigabyte 990FXA-UD3 Mobo (BIOS Version F2 07/15/13)
NVIDIA GeForce GTX 960 (Driver Version 25.21.14.1917, 02/19/19)
