Msi gtx 780 crash problem !!

Hi guys i have already post for this problem but i don't get a serious answer
i have msi gtx 780 twin frozr 3 gb , his HDMI port dont work and when i start games the gpu crash after some minutes !!
and when i go to msi afterburn look what i see :

also when i try to read BIOS file in GpuZ the pc crash and stop working :

i update the bios for this gpu by using nvflash64 in DOS (nvflash64 bios.rom --override -6) and now i have 80.80.21.00.75 version . but the problem dont solved !!

gigabyte h81m
2x4 gb ddr3
i54590s
msi gtx 780 tf
thermaltake Litepower 650W
does my gpu was dead ? i need rebaillage of my gpu or what should be checked exactly ??

How old is the power supply, could just be coming to the end of its life.
