Multi-window video chatting while gaming

Not very hardware related, but a computer question (gaming) nonetheless. I've seen many people play pc games while video chatting with 2 other people, each person in their separate window in opposite corners of the screen. What is a good program/software to do video chatting while gaming like this?

Normally what I seen is they video record themselves (Gamer A, Gamer B, Gamer C) and their gameplay (Gamer A game video on his screen, Gamer B on her screen, etc.) while on 'headsets' chatting over either in-game or one of the voice/chat software (TeamSpeak, Razer, etc.). Then later in 'post-production' one of them edits the video and merges the video game play and places one 'window' in each corner to shwo the players 'video' of them playing, synching the videos, audios, etc. and boom there is your 'youtube gaming' session.
 
In response to Tom (sorry, I'm not aware of a quote function on the mobile site): I appreciate the response, but I was hoping for a more streamlined solution. I am well aware of Skype, but it seems that people simply use a voice/video chat software and then use FRAPS to record the gameplay including the video chat, and then simply compress and upload it. Maybe I am wrong and it is not that simple, but that's why I was asking. But thank you for the answer, should I not find another solution, this will work just fine.
