My brothers ps4 doesnt blow air out the back and makes a lot of noise, fix?

Im wondering if the Nyko cooler would fix this since it attaches on the back. I know he bought a console dont hate me.

If the fan isn't spinning putting one of the back isn't going to fix it. Is it under warranty by chance. What kind of noise does it make? maybe it's just not that hot and doesn't put out a noticable amount of hot air that you can directly feel, but it reality it is.

It makes a grinding / clacking noise and only the small fan on the back near the ports works and spits out hot air. The side fans both intake air just fine.
