My Games Are Constantly Freezing!

well, at the moment its GTA V and Watch Dogs 2 that keep feezing, I want to know ifs either my ram or if its my graphics card, its constantly studders and freezes, its annoying and always happens when I'm 5-10 minutes in the game, somebody please help!
