My pc fan just burned out !

I've never known a fan intended for use in a PC to have anything but a 12 volt rating so I tend to doubt it.

Did the fan work before using the adapter?
 

The fan has been left off for quite a long time because i needed the 3 pin extension lead that was connecting it to the mobo
It was going to set on fire lol it was only on for about 3 seconds if that before i pulled the power cord.
The smell from it was awful and im just glad nothing else seems to be damaged
I mean even if say the fan was stuck somehow it would blow instantly like it did surly ?
 


Even being seized solid shouldn't cause it to draw enough current to burn up the lead wires. If you doubt, just put a finger on one of your fan hubs and 'drag' the spinning blade to a stop; they're made kind of fail-safe that way.

That had to be a straight short to ground of the 12 volt lead. Lucky you plugged it into a power supply molex plug instead of a motherboard header or you'd be looking for a new motherboard. And then probably blow that one when you plugged this bad fan into it!

Oh...just thought...is that why you're using this fan adapter? because the motherboard header didn't work that you had this fan plugged into?
 


The fan is very old its the original top fan from this case

It has a molex connector for the led's and a 3 pin for the fan
I had the molex plugged in some time ago and the fan was not working and the led's were on
Today i plugged the molex in so the led's worked and the 3 pin to the adapter
Then poof
