It started around 6 mounts ago. At first the freezing was from time to time and I just waited 2-3 min and it came back to normal... Now its at the point were I can't even talk on Skype ( video games won't even start) without it freezing and I have to do a hard restart... I asked a lot of friends about this, looked on internet and seen that I'm not the only one that has this problem and that no one has the answer... My PC is old, but I'm not in a situation were I can afford a new one so I need to work with that I've got...
All my drivers are up to date, PC is clean as it gets, I already have put it on best performance...

It sounds like your computer is just getting a little old and things are over heating. Not much you can do about it and these days, There are not many games that run with out PC's.

You have the same video card i used to have before i got my new computer and it broke to the point where i could only play games that used Direct X and most games use Direct X now. But anyway, It just seems like your computer getting old and there not much you can do apart from getting a new computer of replacing the motherboard and all the old components
