My Second Monitor has no signal with GTX 760 2GB

I bought a Galaxy GTX760 2GB, I have a little technical issue I hope you might help me with,
I have a second monitor (Sony Bravia TV KLV-26BX350) connected to the graphics card Galaxy GTX760 2GB, but it has no signal (but it rarely works), and here are the facts and tests I tried:

- Restart the PC (used to work but not anymore).
- Turn off the main power from both the pc and TV.
- Updated the driver.
- Changed the port on the graphics card.
- Changed the HDMI cable.
- Changed the HDMI to Mini HDMI converter.
- Changed the TV.
- Tried a different OS like Ubuntu 12.04.


Here is my current build:
* Mother Board: Gigabyte GA-z77x-UD3H
* CPU: Core I5 3570k
* GPU: Galaxy GTX 760 2GB
* PSU: HEC ACE-580UB 580W ATX12V
* HDD: 1TB HDD X2
* GPU Driver version: 344.75

I hope you can help me with this issue.

If you've changed the secondary display(tried another tv/monitor) and used a known working cable then it's either software or hardware..
focusing on software, it's not something silly like in the Display properties you have hard set to only display one display(I often do this myself deliberately, i have a 60" NeoPlasma as my secondary display).. outside of that, it must be hardware related but that makes almost no sense if your PCs monitor is working in both ports at the back of the video card.

Odd indeed.. seems like software related to me
