My USB Device Keeps Unformatting Itself

I have a 2GB USB drive that I have formatted to FAT32, but the device keeps unformatting itself.
It will work the first few times after being formatted, but then goes to a format that Windows cannot detect.
I then have to connect the device to my Linux computer in order to reformat.

What is causing this and how can I fix it?

sounds like the flash is failing...

UPDATE: Plugged in my USB to my Ubuntu and it says that it was unallocated space.
Now I can't format it? Is the hard drive garbage?
