NAT type stuck at moderate for PS4

I have made a Static IP for my PS4, I tried port forwarding the port that Black ops 3 says is causing the issue (3074 UDP) and it still says moderate. I tried upnp on and off, I tried DMZ. I can't figure it out and it's making me mad haha. Can you tech Gurus help me? I need the NAT type Open. Thanks you in advanced.
 
Solution
if the Verizon modem/router is in it's default configuration, you probably currently have a double NAT.

Try assigning a Static IP to the Asus WAN port and then put that IP into the Verizon modem/router's DMZ.

Finally try either port forwarding on the ASUS or put the PS4 IP in to the ASUS's DMZ.
