Solution
To upgrade your PC to gaming at medium settings @ 1080p, these components are good to go for:
CPU - i5-8400 (or i5-8600K if you have the money. Note: i5-8600K needs aftermarket CPU cooler, e.g Arctic Freezer i32.)
MoBo - any Z370 chipset MoBo will do
RAM - 2x 8GB (total 16GB) 3000 Mhz. You can go with lower speed RAM too if you like to save some money, e.g 2133/2400/2666 Mhz.
GPU - any GTX 1050 Ti (or you can go with GTX 1060 to get high/ultra settings @ 1080p)

Example:

CPU: Intel - Core i5-8400 2.8GHz 6-Core Processor ($178.90 @ OutletPC)
Motherboard: MSI - Z370 PC PRO ATX LGA1151 Motherboard ($129.99 @ SuperBiiz)

Solution
New games at medium settings at 1080p requires more than that CPU and more than a 750 Ti. So in essence you need to build a new computer. Prices are very high right now, so this will be expensive. You can use your 750 Ti to save money, but it will limit game performance.

Alternatively, you can simply buy a used AM3+ motherboard and continue to use what you have. But you'll need to play less demanding games, or at 720p low settings.
