Networking Problem - Ping Spikes

So I'm certainly not an expert in this field, but I've slowly learned more about how to troubleshoot your network since I've been experiencing bad ping spikes whenever I attempt to play my ps4 (multiple games). I have TWC with a 50 download and 5 or 6 upload speed and my ookla speedtest shows the speeds varying between 20-40 download but 0.15-5 upload. I'm assuming this is why I'm experiencing the ping spikes. However, the 0.15 mbps upload speed period is short and then returns to around 5. I've forwarded my ports and adjusted the QoS to allow my ps4 to have optimal speed to see if that would help but it didn't help much at all. In call of duty I'm usually experiencing a 30-50 ms ping, but sometimes it will jump up to between 130-400 ping the max allowed to be shown. The router is a NETGEAR R6250. I hardwired the router into the computer and pinged the router when I realized I was having issues in game. The ping test came back <1 ms the majority of the time obviously, but had ping spikes every 10 seconds or less with pings between 3-15 ms. Is this showing me that my router is the issue here or is there another explanation for my lag spikes? This issue comes and goes because sometimes I won't experience any lag spikes all day. Currently, the lag spikes in the ping test have gone away and the pings are consistently less than 1 ms as they should be, but this has been a continual problems with it coming up most days. I'm just trying to pinpoint the issue here so any advice is appreciated.
