New GTX 1060 display driver error and games crashing

Hi, Im having issues with a GTX 1060 GPU 1 month old.

To start the last GPU died with black screens so the card was replaced with the 1060. The card worked great until this Friday, every game I play crashes with display driver nvlddmkm has stopped responding (event ID 4101). Also a message saying the application was blocked from accessing the graphics hardware (Event ID 4109).

I have done all the usual tests:
Format and clean install of win 10 then run updates
All drivers updated
Upgraded the Bios
Changed PCI-e slot
Tested in 2 other PC's

I ran MSI Kombuster and the program crashes when tesslation test. 2D test in seperate program works fine, 3d test 2FPS then CTD (both Even ID's in log).

Could another sytem part have caused the fault on this card or is the card just faulty?

Specs CPU FX 3600
Board: 970a-ds3p
Ram 12GB
Coolmaster 650w PSU (less than a year old)

If anyone could shed some light on this then it would be greatly appreciated. Thanks :)

Yes the GPU is probably dead but it's a little strange to have 2 GPUs that died within a month. It may be a coincidence but it also may be something else like a bad PSU, that killed both of them.
If that GPU doesn't work on another PC then it's dead/bad. It's not unlikely to get a new GPU that dies within a month. That said a cheap/bad/damaged PSU with bad voltage output may have fried both your GPUs. Give us your exact PSU model. You may have to get a new and better PSU. If your current PSU is indeed bad, your entire system is in danger.
