Odd USB issues in Windows 10

So, I've been running my current setup for a couple of years while now, and I've recently come up against an odd and annoying issue regarding my USB ports on my build. I've got 6 ports in the usual position on the mother board (4 x USB 2.0 and 2 x 3.0), an additional pair built into the top of the tower, connected to the USB pins on the mo/bo, and I've also got a multi media panel on the front, comprising of a multi card reader and an additional 2 USB ports, again, connected via USB pins on the mo/bo.

In the last few months, the front panel stopped working. At the same time, my USB 3.0 ports have also ceased working, and now, my top ports have also stopped working, too. I bit the bullet, and purchased a new front panel to see if that would help, which connects tothe mo/bo via a 20-pin cable as it has USB 3.0 ports. This is where things get odd:

Although the front panel does not read any media, an attached USB thumb drive will flicker to show some activity as the PC powers down. I've checked device manager, and there's nothing there with a yellow warning triangle. In the course of my research, I've read that the XCHI settings in the BIOS can cause issues in Windows 10, so I've tried with XHCI set to auto and smart auto (currently on smart), and with XHCI and EHCI hand-off enabled and disabled (both currently disabled). The top panel shows no activity whatsoever, however, this will not be an issue if I can get the front panel working!

Would the card reader require drivers? It's a generic, unbranded model that didn't come with any discs, or even a web address (5.25 Media Dashboard 525F20, if that's any help!)

Any help would be gratefully received, as this is starting to drive me nuts!!!
 
Ive only got the front panel and the hdd attached to the SATA ports (although in the BIOS, only the HDD registers, surely the card reader part of the panel should show here?).

Other than that, there's a geForce 740 GT attached, which doesn't require an additional power supply beyond plugging into the PCIe slot.

As far as I can tell from old receipts, the power supply is an Artic Cool 850W ATX modular thingy.

No the card reader wont appear in the BIOS. But it should appear under disk drives in device manager. But going by the pic, it's not there

Whats the make/model of this multifunction card reader?? Do you have a spare desktop you could test it in to see if it works in that?

Anyway, a bit of an update:

After going down the devices listed under USB in the Device Manager, right clicking and checking for updates (something I thought I'd already done!), the card reader's USB 3.0 and the two rear USB 3.0's are now functioning.

The card reader itself is not working (the activity LED is constantly on, if that helps), and if I plug a USB drive into the top ports, I get this error:

What you could try is delete the USB entries under USB controllers in device manager then reboot. But if youre using wireless you'll need another keyboard after you delete the entries

Or go to device manager / select something, select action and click on scan for hardware changes.

See if it picks it up

Look in disk management. Does the reader appear here at all??

And if it says D. Is something else D?? Maybe something (a drive letter) is conflicting with the DVD's driver letter (if you've got one)
