Where did the phone come from?

In the US, only Verizon is mostly unlocked. All other carriers are typically carrier locked initially - but can be unlocked upon request, once the device balance has been paid.

I just recently got an iPhone and the prior carrier was Consumer Cellular. It was owned by a family member previously. At first I was concerned, but after calling Consumer Cellular, letting them talk to my relative, then I asked them to unlock the device and they did it for me and the whole thing only took 3 minutes. I didn't get any glowing confirmation on the screen that it was successfully unlocked, but when I went to activate on a different carrier it worked perfect. The main criteria in my scenario was that the device had to be owned for a predetermined length of time and I believe federal regulation requires that they unlock the device.

"Free" upgrade, you need to read exactly what is in the statement about that upgrade. A lot of times the "free" stuff is only free is you use it some period of time on the service or you will be changed for the cost of the phone.
