[SOLVED] OS X crashes on sleep

Hi guys, i have a hackintosh running Capitan 10.11. I have two problems. One of them is really annoying. My account keeps logging me out when i am in the middle of things. I had the hackintosh setup before with intel hd4400 graphics but it didn't have this bug. P.S: I have re-formated the ssd since then. I used windows for a while until i went back to hackintosh. I tried countless times to get hd 4400 working but it was no use so i put a 750 ti in. Did a fresh install and downloaded the drivers. It is working fine except for this bug. Any solutions?

Next, this is the sleep bug. This didn't work when i had hd 4400 and it still doesn't now. I was wondering if anybody could analyse the problem and see what it is. I am quite tech savvy so i can help with things. Any help would be greatly appreciated!
