Question Over and under-voltages on the motherboard.

Hello,
So I would normally ignore this and assume it's just inaccuracy of the software, but the voltage levels displayed by HWMonitor are very bizarre.

Should I be worried? Recently I've been experiencing blue screens regarding my HDD paging, I assumed the connectors weren't well connected, so I double checked that and it doesn't seem to be the issue. Additionally, both the PSU (350 Watt, note that neither my CPU or GPU [GT 220] are powerful) and HDD are fairly old, so I should probably consider replacing them. How urgent is it to replace the PSU? Can other components suffer from this if I don't?

So I measured the voltages manually, and it's a relief:
12V -> 11.78V
5V -> 4.94V
..
So everything seems fine.

I'm still curious as to the reason why the software measured values are so off?
