Overclocking stability issues

Recently I overclocked my CPU to 4.0GHz from 3.5 and I've noticed some problems that I don't know what they are or how to fix. They include trouble booting up, i.e. computer starts up 2-3 before reaching the BIOS start-up screen and going to the login screen, or crashing after two continuous days of being on and then having issues with the computer running. It'll turn on but no screens will show up, so I have to spend like 15 minutes turning it on and off to get it to work. I upped the voltage to the CPU a little in hopes of solving this issue but no luck as of yet. Is this strictly an OC issue or is there something else I'm missing?

My build- AMD FX-6300 3.5GHz 6-core processor
Gigabyte GA-970A-UD3 ATX AM3+ Motherboard
HIS Radeon HD 2GB 7850
Mushkin (2x4GB) 8GB 1600DDR3
(3) 7500RPM HDD's ([2] 1TB, [1] 500GB)
Cooler Master Hyper 212 EVO Heatsink
(5) 120mm case fans
(1) 140mm case fan
NZXT source 210 Elite case
CORSAIR Enthusiast Series TX750M 750W ATX12V v2.31 / EPS12V v2.92 80 PLUS BRONZE Certified Semi Modular High Performance Power Supply

Solution

Undo the OC and see if the issue continues.
It is likly related to the OC, but double check your ram and graphics card to be sure. Power consumption heightens with OC so be sure you has a large enough supply to manage the OC with all the other components

My PSU is a 750W so I have more than enough wattage

Undo the OC and see if the issue continues.

Solution
So I sort of solved the issue but another one came up. The shutting down was because the ATX cable to the mobo wasn't plugged in correctly. Now, it give me a blue screen then shuts down. I'm assuming that's because my OC isn't stable. The 2-3 rebooting still happens as well. I just bumped my core voltage to 1.380V to see if that'll solve the instability but can't seem to figure out why it take 2-3 tries to successfully boot up.
