Pc display issues

Ok I have some strange issues with my new pc.
I have multiple display issues:
1. When returning to pc after it has went to sleep it will appear very fuzzy. Rebooting fixes this.

2. The pc will crash and have a tdr failure error. This happens randomly.

3. The pc will crash usually without error as it usually freezes up before getting to any error. The most frequent time this happens is at the very start of a league of legends game right when it switched from load sceen to the game map. I have had it happen during a game but its rare. I have had it happen in other games but much more rare. I can play much more demanding games such as the new deus ex and rarely have issues.

So far I have updated the bios and installed a new graphic driver using a clean install so it should be up to date. Also went into registry and disabled ulps and this did not help.
Also tried using frame limiter off and on. Also turning crossfire off did not seem to make any difference.

I do not think this is likely hardware related as the pc is under light load when this usually happens and I can play more demanding games and dont have the issues that often.

My setup is I5 6600k stock clocked
480 cf 4gb
16 gb 2400 ram
Win 10 64 bit

You say turning of CF doesn't help. Does that mean you turned it off through Crimson alone or also removed the card and played that way?

What is the complete make and model number of your PSU?

What does MSI afterburner say during your gaming sessions? I understand this most likely isn't hardware related. To look at the patient's chart is only good manners.
 
Crossfire turned off only in amd settings.
I bought the pc off newegg because it was very good price imo and not sure what psu is in it. Just says 600 watts.

I uninstalled afterburner so I do not see it. I dont see it being high usage in amd settings ot windows performace monitor.
