PC Freezing & Screen going black & changing resolution?

I've only had this problem for a few days, but it's getting annoying. My PC just seems to be so messed up right now! I only got it 5 months ago and I don't want to let it go to waste. I have never had any problems with it until this point. It started as one game on my PC would really lag and my PC would be really laggy afterwards (the game is GTA 5) for about 5 minutes then it would be normal. Over the past couple of days I've been noticing my PC have more and more problems stacking up. Basically what happens is if I am in a programme or something my PC randomly freezes for 10 seconds or so and after that the screen goes black for 10 seconds or more and then when it comes back on the screen resolution usually changes to like the lowest setting. I'm pretty sure my GPU (GTX 760) can handle my one 1920x1080 monitor pretty finely as it has been doing for 5 months. I have another monitor but I just use my Intel Integrated graphics for it. My CPU is an Intel Core i5 4690 and I have 8GB RAM. I've tried running several scans on many different programmes and all come out fine (McAfee, CCleaner, Malwarebytes, Auslogics etc). If someone call tell me what the hell is wrong with my PC I would be very grateful because it's hard to live knowing the PC I paid $1000 for only 5 months ago isn't working properly.
 


I have 550W Corsair VS, The temperatures are about 50 Degrees Celcius and I don't really know what the gpu heatsync is but I recently cleaned everything with a cloth.
 
canned air is the best thing, for future reference. does the 760 have dual outputs? It is more than capable of running both monitors simultaneously. I would not recommend using the integrated while using a dedicated GPU. possible driver conflicts, resource requests, etc. I would disable the onboard and remove the intel graphics driver. Run with one monitor to see if this shows any result.
