Question PC Games running at 60fps but not smooth and occasional grainy/blurry textures

I'll keep this brief and go into any necessary detail in the comments. PC ran amazingly for a long time. Then all of a sudden last summer, resource-intensive games that looked and played perfectly at full settings took a huge drop in quality microstuttering and showing occasional grainy textures during motion. Games will run at 60fps but clearly not be as smooth as that. (The only potential cause I remember is torrenting Black Ops 1 around that time). To this day none of my hardware is throttling.
So far I've:
- clean installed Windows 10 on a new SSD
- reinstalled drivers
- used different PCI cables from PSU
- tried different HDMI cables and monitors
- updated Windows and BIOS

What seemed to help a little was using O & O Shut Up to remove Telemetry and some other Windows bloatware. Oddly enough the footage of Street Fighter 6 I recorded looks the way it should. Although, it didn't look that way as I was recording it. My current monitor's refresh rate is 100Hz.

Latencymon did return this: "Conclusion: Your system appears to be having trouble handling real-time audio and other tasks. You are likely to experience buffer underruns appearing as drop outs, clicks or pops. One or more DPC routines that belong to a driver running in your system appear to be executing for too long. One problem may be related to power management, disable CPU throttling settings in Control Panel and BIOS setup. Check for BIOS updates."
This has been driving me crazy for almost a year, any ideas?
