PC not performing consistently

I have a Lenovo Y50 (GTX 860m, i7-4710HQ, 8GB RAM). One of my favorite games is Dark Souls, so I installed it. Since it's a straight console port it's locked at 30 fps, but that's not the problem. Once I installed it for the first time on this PC, I was pleased. It looked beautiful at 1080p on this IPS screen, and I could run it flawlessly locked at the 30 fps cap.

I played it on and off for about a day.

When I played it today, the problem started. Randomly after loading my save, the framerate was unstable. It fluctuated from 10-30fps, which is pretty much unbearable. I was in the exact same place I'd been when it was running effortlessly locked at 30 fps, looking at the same things, with the same settings, but for some reason it wasn't running as well.

Perplexed, I cut the rendering resolution from 1920x1080 down to 1366x768 in DSfix. That didn't affect the framerate at all, so I suspected a CPU bottleneck. An i7-4710HQ should never bottleneck a GTX 860m under normal circumstances, so I suspected thermal throttling. However, the temperatures were perfect. My Y50 was merely slightly warm to touch, and the temperatures never exceeded 65c in-game (straight PS3 ports aren't very demanding XD).

Since the laptop was already plugged in, next I disabled Nvidia Battery Boost and set my CPU to max frequency, and killed off background tasks. It didn't help at all.

Right now I'm left with a laptop that I know can run Dark Souls maxed at 30+ fps, but it simply won't, despite the fact that I have a clean driver install, normal temps, and saw it performing correctly just a few hours ago. I have no idea what's going on, or how it could suddenly change like this. Any ideas?

I don't have other games installed right now, so it'll be several hours before I can test to see if anything else is performing correctly.
