PC posts and then display loses signal--PC remains ON

Hello all,

Any help would be appreciated as I have searched my issue and have been unsuccessful in finding any information.

As of late, maybe 1 out of 10 boots, my PC will post (to the screen giving the option to go to BIOS) and then right after, the display loses signal. The PC appears to remain on. I can see inside the case and the temperature gauge and all the lights seem to be functioning normally. However, nothing I hit on the keyboard (or unplugging and plugging back in the monitor) seems to make any difference.

At this point, I am forced to do a hard reset. When rebooting the PC, it posts again with a notice that it is checking and repairing partitions. After a few short seconds, it fully boots as normal to home screen and everything is fine. Again, maybe once every 7 to 10 boots does this issue occur.

Cpu: i7 6700k
Motherboard: msi z170a gaming m5
Psu: evga 750 watt 80 gold plus
Ram: corsair vengeance
Ssd: 250 I forget
Hdd: 1 terebyte western digital blue

Don't know if it relates at this point, but a while ago the hdd was making pronounced struggling noises. When this happened. The computer failed to boot at all. Thing is nothing is installed on it. It's completely empty. Happened twice maybe 6 months ago and not since. Has fairly normal sounds now.
