[SOLVED] pendrive wont format under linux as well as windows

my college issued me a pendrive when i joined my college. it used to work fine and gave around 20MB/s write speeds. i was trying to create a bootable driveusing this pendrive and i saw i was getting terrible speeds . so i went into diskpart and cleaned the drive. since then i am not able to format the drive or create a partition table on the drive . both in linux as well as windows. fdisk allows me to create partitions but they are not made .
i ran usb disk info extrator and it gave out this info

Solution
Ok, when you've used two independent ways to format the drive and both fails, then it must be the pen drive that's in bad condition. I'd toss it and buy another one.
