Piezo 3 wire to jack

I am making a guitar piezo pickup(contact mic) but i have a 3rd wire on it(piezo) and i want to wiring piezo to 3.5mm jack but i dont know where to use the 3 wire piezo ..?
Can you describe me the wiring of piezo to 3.5mm ..?
