Question Please help me with GPU error

I just built a PC with a

650w psu,
i5 2400 optiplex 990 motherboard
some full tower case (idk what one)
pcie riser cable and an
RX 570 8 gb phantom oc

When I first set up the gpu and installed drivers it worked fine and came up in pcie x and device manager. I reset my PC and plugged my monitor into my gpu and it booted fine and it said it was ok in device manager. But when I launched a game my PC crashed. I turned it on again it said there was an error with the gpu in device manager and windows had disabled it.

I reset my PC and I haven't had any signal from my gpu since, if I plug my monitor directly into my motherboard output and launch it says that there is a gpu detected and I need to plug my monitor directly into my gpu and then restarts but when I plug into my gpu it still turn on and I get fans and the lights on my keyboard work but I don't get a signal.

Can someone please help me resolve this?

Update:
I got it to launch with the gpu connected and with the monitor connected to the motherboard output and it says the gpu is working but I still get no output though the actual gpu and I can't actually use it without the output going through the actual gpu
