Possible faulty GPU question.

Hello, I am in massive need of answers right now and would very much appreciate your help. So, my problems started all of a sudden, about a week ago. I started seeing some kind of black rectangles on my screen like this: http://i.imgur.com/FL9ZbvN.jpg . I thought my GPU's life might come to an end soon, but I hoped not that soon. These rectangles usually disappeared after a few seconds and I could mind my business on the PC and play games without a problem. They didn't appear very often. Today my image started freezing whenever I tried to play anything. I tried it in furmark, still freezes, crashes, and I have to restart my PC. This is what happens: http://i.imgur.com/1HWZQil.jpg. If you used furmark, you know that there should be a furry circle that keeps spinning. My furmark crashes after few seconds. My question is: was I right? Is my GPU almost gone?


P.S.: Today I moved the components of my PC from and old case to a new one. I'm no expert, but I manage with PC's pretty well, since this is what I study. I don't think my image and GPU crashes because of this, of me not connecting something right, but I might be wrong, and wanted to specify this too. Please help!
 


I underclocked the GPU but weird rectangles still appeared while testing in furmark and after that, the image just froze again and the only thing I could do is to restart. So is it dying? It sucks cause I had it for about 3 years only.


P.S.: I underclocked it even more, to the minimum and although I had 8 frames with 100% load in furmark, it didn't crash at all. So by your theory it is dying. 🙁
