Possible R9 380 or PSU Fail?

Hello.

I have had this issue before, but most recently it has occurred in Watch Dogs 2. Within 60 seconds of starting the game (the first scene, not the main menu), the screen will freeze, eventually turn black, and either close the program or give the dreaded "application is blocked for access to graphics hardware" error.

Here are my event logs from the most recent crash:
Event 4109 - Display - Application WatchDogs2.exe has been blocked from accessing Graphics hardware.
Event 4101 - Display - Display driver amdkmdap stopped responding and has successfully recovered.

I have tried:
- Verifying game cache (verified successfully)
- Way underclocking my GPU (down to lowest possible settings in MSI afterburner)
- Setting the graphics quality to low, running in windowed, in 640x480 res (still crashes!)
- Checking for driver updates (latest)
- Numerous reboots

I have never had this issue in CS:GO, ARMA III, or any other games except for Tom Clancy's Blacklist. It seems to be an issue with AAA titles (suspiciously only Ubisoft titles :/)

I have read that the black screen could be a sign of a failing PSU, but my PSU really shouldn't be under load, it is a 550W that I purchased around 2 years ago, powering a 3.6 GHz i5-6600k, a Sapphire R9 380 4GB, an H110i v2 cooler, and an assortment of peripherals. A power supply calculator has informed me that I need ~440W.

Thanks.

Solution
Honestly based on that I think your GPU may be the issue, not the PSU.

Did you buy it new? Do you have a friend whom you can borrow theirs to try?

Actually I believe it's an EVGA 600B, if not a 500B. I thought it was 550W but I was mistaken, I'm sure it was a bronze-rated EVGA

Sorry misread that, see reply above

Ok not the best PSU but it should drive that system as long as you're not overclocking anything.

While I do doubt it, do you have access to another PSU to test?

Nope. Would it help testing if I lower the power limit in MSI afterburner? Or underclocked my CPU?

Sorry, I don't own 3DMark, any suggestions for a free high-performance benchmark?

Also Rainbow 6 Siege works fine, and I don't understand how Ubisoft could be at fault, I just think that Blacklist is probably the second most demanding game I have installed.

3DMark Firestrinke is free, you can get it on steam or here:

Scroll down a bit on the right "3dmark basic edition"

Alright, I ran 3dmark and my computer froze twice on the main screen. I noticed at the bottom it was "loading graphics card info", perhaps querying my card is causing the issue? Though I've never had any issues with speccy. My computer froze for several minutes both times until I held the power button. Cursor froze in the spinning animation, so it was definitely a complete hang of the system.

Bought new, but don't know of anyone who would let me borrow theirs. Oh well, at least Vega is tomorrow, maybe I'll be in the market.

Thanks for your help.

Yeah, I'm also pretty convinced of the GPU causing the issue. It's given me problems for a while, the fans would crap out and temps would go above 100C, I think something might've burned out.

That definitely would burn something out. I think then your answer is much clearer now.
