Possible SSD failure

Help!

Every few months either window 10's files get corrupted or my PC starts to fall to boot. Each time I format my OCZ (basically Toshiba) ARC 100 SSD and reinstall Windows 10... Which is tedious to say the least.

This time, my PC won't open USB pen drives and Asks to format them, when they're fine on all other computers. I've used System File Checker, and there are errors but it couldn't be auto corrected. I can't open device manager nor can I run mdsched.exe

When I plug in my old HDD, everything works fine. Checked memory (which I suspected could be the issue for the continuing corrupted files... But it's all fine.

OCZ health check reckons my SSD is fine

Wtf?

I suspect I need a new SSD? Or are there other possible causes for the window for corruption?
 
An unstable OC or bad/failing RAM can also corrupt an OS installation. However, you tested RAM already. Does HDD stay consistant and files remain intact? The OCZ tool likely is checking SMART dsta and wear leveling. However, these are not 100% accurate for reliability. For reference, I had an original OCZ Revodrive SSD replaced 3 times. #1 was bad from start, 2 was fine for awhile then started corrupting data, 3 was fine for a year or so. Then after installing in another PC, it started corrupting data as I needed to keep reinstalling soundcard drivers and system files. Number 4 is still working as secondary drive.
